Care Partner Consultation

  • Guided discussion to problem solve care challenges

  • Written summary of key points 

  • Up to 3 participants

  • $75/hr (in person or via video call)

Care Planning

  • Personalized assessment of needs and preferences

  • Holistic support for living well with dementia

  • Addresses needs of care partners, family members, and person living with dementia

  • Written summary of goals and supporting resources

  • Recommend in-person, but virtual meeting also available via video call

  • $75/hour

Personalized Enrichment Visits

  • Engaging one-on-one activities designed to meet individual interests and abilities

  • Offers purpose and connection, which often leads to moments of joy!

  • Available in private home or memory care

  • Enrichment visits available in Bend only

  • Care partner can choose whether to participate or take a break

  • 1-2 hour visits at $50 per hour

Travel fee of $10 will be added for Central Oregon consultations outside of
Bend, Redmond, Sisters, or Sunriver.
Cost of travel outside of Central Oregon to be negotiated.
